When you schedule an appointment with more than one participant, it only shows in the activity log for the primary participant.

The secondary (and/or all other participants) do not show the appointment on their individual record

For example: you book an appointment for a husband and wife. They have separate Salesforce person account records. It only shows the upcoming appointment in the Activity Log of the primary participant.
